Looks like a head crash, or some other physical failure of the hard drive
itself. I'd suggest you replace the hard drive, reinstall, mount the old
hard drive as a second one, and try to recover whatever data you can.

The old drive might be usable after reformatting (if it has been able to
remap the bad sectors) but personally I wouldn't trust it.
