The configuration you have in your config file does not stop you from get
blacklisted, but they are protecting you from

receiving emails from the domain which are listed, Your server check
those site when processing incoming emails and if any domain listed there
it will rejected all email from that domain.

So if your server send out a lot of emails without following mail servers
standard to specific or multiple emails accounts in a specific servers
(Domains) it will get blacklisted. This can be caused by:-

-Mail server be compromised
-Mail account(s) in your mail server be compromised, through client
software such as outlook or through web-mail
-Computer under your domain be compromised.

*So check your mail queue to see if there is any email account which send a
lot of emails to a specific domain, or go through your mail logs file.*

Note: Before asking to be removed from the list you have to clean your
server or Pcs which are compromised and also change password of compromised
email account .
