TLS certificates are misissued every now and then.  There are ways to 
be protected against a misissue.  Each of them has advantages and 
disadvantages.  Here's where it gets complicated.  There was a BGP 
attack about two months ago which led to a TLS certificate being 
misissued by Let's Encrypt: https://www.elandsys.com/r/84295
